Adopt-A-Highway application forms

Fill out the desired form and e-mail it to the appropriate regional coordinator. If e-mailing is not an option, you may also send the form via U.S. Postal Service.

The following documents are in PDF format. They require Adobe^® Reader^® version 7 or higher.

• Clean-up report form (DT2214)
• Freeway/Interstate/Interchange work plan form (DT1207)
• Notice of abandonment (DT1205)
