Becoming an organ donor

Anyone can be a potential organ and tissue donor. The anatomical Gift Act in s.157.06(2) requires persons 18 or older to indicate on your driver's license or ID card if you wish to be recorded as a potential donor.

To indicate that you want to be a potential organ or tissue donor, complete an application for a Wisconsin driver license/identification card form MV3001 PDF:

  • Mark the organ donor question "yes."
  • Take the application to a DMV service center.
  • Sign and date the reverse side of your driver license or identification card.
  • Specify all or only specific parts/organs 

An orange sticker can also be placed on the license to alert medical personnel of your wishes.

If you decide you want to be recorded as a potential donor after you have received your license, sign and date the reverse side of your driver license/identification card:

  • Specify all or only specific parts/organs
  • When you renew your driver license/identification card mark "yes" on the application to the organ donor question

If you wish to amend the donor statement, apply for a duplicate driver license/identification card:

  • Pay the required fee
  • Sign and date the reverse side of the driver license
  • Specify all or only specific parts/organs

If you change your mind about being an organ or tissue donor, cross out the donor statement on the reverse of the driver license/identification card.

The Division of Motor Vehicle does not keep an organ-donor registry. DMV records only indicate potential donors. The most important step to having your donation wishes fulfilled is telling your family what your wishes are.
