IRP application schedule A

The WI IRP Amendment Schedule A - form MV2914 is used by parties that have an established WI IRP account. The form is used for the following reasons:

• Adding a vehicle to your account - complete the Additions section
• Add and delete a vehicle - complete the Additions and Deletions sections
• Changes in existing vehicle information (i.e., unit #, vehicle identification #, titled owner, etc.) - list old information, including plate number, in the Deletions section. List new information in the Additions section
• Vehicle weight change (i.e. 32,000 to 54,000 lbs) - identify vehicle and new weight
• Replacement cab card or plate - fill out the Replacement Cab Card/Plates section and provide vehicle information in the Deletion section

Complete the form and fax it to (608) 267-6886. The application will be processed and billed the following work day, and you will receive by fax a temporary IRP vehicle registration for immediate operation.

You have 3 work days from the date of operation to cancel or change an application. The unused credential must be returned.
